What to Know
Life is full of events, but only the major ones, like getting married or having a child, give you the opportunity to make benefit changes outside of the new hire and Open Enrollment periods.
How to Make Changes to Your Benefits
If you have a qualified change in status and wish to change your benefits, you must make that change within 30 days of the life event. To make benefit changes, log in to your Workday account or open an HR Ticket in JSM. If you miss the 30-day window, you won’t be able to change your benefits until the next Open Enrollment.

What Benefits You Can Change
For most life events, you can change your medical, dental, and vision coverage. If you have a Health Care Flexible Spending Account (FSA) or Dependent Care FSA, you may be able to change your contributions. You may also be able to add or remove coverage for life and accidental death and dismemberment (AD&D) insurance.
Qualifying life events and the eligible changes are determined by the federal government and the benefit plans. The changes you make must be related to the life event. To see what benefits you can change, select your life event below.
Birth or Adoption
Congratulations! This is an exciting and rewarding time for you. While it will pose a lot of new challenges and questions, being prepared for what’s ahead can allow you to enjoy the millions of precious moments as they occur.

Medical, Dental, and Vision
You may enroll yourself, your spouse, and/or the child, or you may cancel your coverage if you enroll in your spouse’s group plan. Your coverage or your benefit change is effective the date of the birth, adoption, or placement for adoption.
Flexible Spending Accounts
For the Health Care Flexible Spending Account (FSA), you may enroll or increase your contributions for the remainder of the plan year, or you may decrease or revoke your contributions if you enroll in your spouse’s medical plan.
For the Dependent Care Flexible Spending Accounts (FSA), you may enroll or increase your contributions for the remainder of the plan year.
Life Insurance
When you add a child to your family, don’t forget to review and update your life insurance coverage. You can add your child as a beneficiary, change your coverage level, or purchase additional coverage for your child. You may find insurance calculators helpful as you think about how much life insurance you should purchase. You may also want to consult your financial advisor.
Voluntary Accident and Critical Illness Insurance
You can enroll or make changes to your voluntary accident and/or critical illness insurance.
Beneficiary Designations

- Either apply for your child’s birth certificate at the hospital and indicate that you want to apply to have a Social Security number assigned, or complete an Application for a Social Security Card (SS-5) form and mail it or take it to your local Social Security Administration office.
- Review and make changes to your stock beneficiaries through E*Trade.

Marriage or Domestic Partnership
Whether you’re getting married or beginning a domestic partnership, you’re entering into a bond of love and trust, and with that comes several matters to be aware of: financial stability, health and life insurance coverage, and legal issues.
Note: Coverage or cancellation is generally effective as of the first of the month following your election-change request.

Medical, Dental, and Vision
You may enroll yourself, your new spouse, and any eligible dependent children (including your spouse’s/domestic partner’s children); add your new spouse and any eligible dependent children (including your spouse’s/domestic partner’s children) to your plan; or cancel your coverage if you enroll in your new spouse’s group plan.
Flexible Spending Accounts
For the Health Care Flexible Spending Account (FSA), you may enroll or increase your contributions for the remainder of the plan year, or you may revoke or decrease your contributions if you or your dependent(s) enroll in your new spouse’s health plan.
For the Dependent Care Flexible Spending Account (FSA), you may enroll if you gain an eligible dependent and your spouse is employed, disabled, or a full-time student; increase or decrease your contributions for the remainder of the plan year if your expenses increase/decrease as a result of the marriage; or stop participating if your spouse is not employed, disabled, or a full-time student.
Life Insurance
When your estate increases (buying a home), your salary increases, or you add dependents, your life insurance should also increase to protect those who depend on you. If you’re getting married or formalizing a domestic partnership, this may be a good time to review your policy and increase your coverage.

- If you legally change your name because you get married, apply for a corrected Social Security card with your changed name. You’ll need to either complete an Application for a Social Security Card (SS-5) form and mail it or take it to your local Social Security Administration office, or visit the Social Security Administration website or call 800-772-1213. To speak to a service representative, call Monday–Friday, 7 a.m.–7 p.m. Eastern Time. If you’re hearing-impaired, call 800-325-0778.

Losing a Loved One
Whether expected or unexpected, the death of a loved one creates a wide range of emotions and life changes that you’ll need to address. As you begin to get your loved one’s financial affairs in order, one of the first things you should do is review his or her benefits package. You should also make whatever changes are necessary to your own benefits.
Note: Termination of the deceased dependent from Exelixis benefits is effective as of the date of death. If your coverage was under the deceased dependent, and you (the Exelixis employee) then enroll your remaining dependents and yourself in Exelixis benefits, your coverage will go into effect on the first day of the following month.

Medical, Dental, and Vision
If your coverage was under the deceased dependent, you can enroll yourself and any eligible dependent children in the Exelixis plan.
If coverage is under the surviving employee, termination of the deceased dependent from coverage will apply with no other changes allowed.
If coverage was under a deceased Exelixis employee, qualified beneficiaries (those enrolled in the plan on the day before the date of death) will be eligible for COBRA for a 36-month period following the date of death.
Flexible Spending Accounts
If coverage was under the deceased dependent, you can enroll in the Exelixis plan.
Life Insurance
When you lose a loved one, you’ll likely need to go through the process of filing an insurance claim. Get more information about the claim-filing process, including instructions on preparing your claim, a claim-filing checklist, and ways to connect with an advisor.

Social Security and VA Benefits
Social Security: Contact your nearest Social Security Administration office for information on survivor benefits. A deceased person’s spouse, minor children, or even divorced spouse may be entitled to benefits. However, Social Security laws change frequently, so check with your local Social Security Administration office or call 800-772-1213 for additional information.
Be prepared to provide:
- The death certificate (or a certified copy)
- The Social Security number of the deceased
- The decedent’s latest W-2 form or tax return
- The name and address of the decedent’s employer
- Social Security numbers of spouse and minor children
- Birth certificates of spouse and minor children
- Marriage certificate
- Divorce papers, if applicable

Retirement Savings Plan
As a 401(k) plan participant, you’re required by law to receive a Required Minimum Distribution (RMD) from the plan at a designated age determined by the IRS. To learn more, review the 401(k) Summary Plan Description (SPD) located on the 401(k) Retirement Savings Plan page.

The Social Security Administration offers a wealth of information that can help you plan for retirement:
Life and AD&D Insurance
Your basic and voluntary life insurance and accidental death and dismemberment (AD&D) insurance ends on your last day of work. You have the option of converting all of these plans, except the voluntary AD&D plan, to an individual plan, subject to certain conditions. You’ll receive conversion information directly from Prudential.
Voluntary Accident and Critical Illness Insurance
If you’d like to continue your voluntary accident and/or critical illness insurance coverage beyond your employment at Exelixis, fill out and return to Prudential the continuation forms (included in the Additional Resources below) within 30 days of your last day of work.
Medicare
When you reach age 65, you’re eligible for Medicare. Medicare eligibility doesn’t mean you can’t be enrolled in Exelixis benefits. Refer to the Medicare-Specific Resources section below to learn more. If you enroll in Medicare and want to waive your Exelixis benefits, you can do so through Workday. Separation or Divorce
If you’re in the midst of dissolving a relationship through separation or divorce, here are some benefit-related next steps you may want to take.

If you get separated or divorced, you may enroll yourself and any eligible dependent children if you’ve lost coverage under your former spouse’s group plan, cancel your former spouse’s coverage, or cancel your dependent children’s coverage if they’ll enroll in your former spouse’s group plan coverage.
Flexible Spending Accounts
For the Health Care Flexible Spending Account (FSA), you may decrease or revoke your contributions for the remainder of the plan year, or you may enroll or increase your contributions if you lose coverage under the former spouse’s health plan.
For the Dependent Care Flexible Spending Account (FSA), you may enroll or increase your contributions for the remainder of the plan year if your expenses increase as a result of the divorce/legal separation, or you may decrease or revoke your contributions if the change eliminates the need for care.
Life Insurance
When your estate decreases (selling a home), your salary decreases, or you remove dependents, you may also want to decrease your life insurance. If you’re getting divorced or separated, this may be a good time to review your policy and adjust your coverage.

For the Health Care Flexible Spending Account (FSA), you may decrease or revoke your contributions for the remainder of the plan year if you gain health coverage eligibility under your spouse’s plan.
For the Dependent Care Flexible Spending Account (FSA), you may enroll if your spouse becomes employed or a full-time student, or increase or decrease your contributions for the remainder of the plan year to reflect changes in the cost of coverage.

Loss of Coverage Elsewhere
If you and/or your spouse become ineligible for your spouse’s or parent’s employer-sponsored plan (e.g., termination of employment, reduction in hours, a dependent turning 26), you may want to change your Exelixis benefits coverage.

You may enroll yourself and your eligible dependents if you and your eligible dependents were covered under your spouse’s plan.

For the Health Care Flexible Spending Account (FSA), you may enroll or increase your contributions if you or your spouse lose coverage under a spouse’s or parent’s health plan.
For the Dependent Care Flexible Spending Account (FSA), you may increase or decrease your contributions for the remainder of the plan year to reflect changes in the cost of coverage, stop participating if your spouse is no longer a full-time student or no longer gainfully employed or actively searching for gainful employment, or enroll or increase your contributions if your spouse loses dependent care FSA eligibility.

Dependent Turns 26
On the last day of the month of your child’s 26th birthday, that child will be automatically removed from your Exelixis benefits, unless the child is listed as disabled in Workday. For more information, review How to Edit an Existing Dependent. If you have questions, open an HR Ticket in JSM.
Note: Coverage or cancellation is effective as of the last day of the month of your child’s 26th birthday.
Medical, Dental, and Vision
Your ineligible dependent will be removed from your coverage. The ineligible dependent will be eligible for COBRA if the plan is notified of the qualifying event in a timely manner. A COBRA packet will be mailed to your home address as it’s listed in Workday.
Flexible Spending Accounts
You may decrease or revoke contributions for the remainder of the plan year for your dependent who loses eligibility.
Life Insurance

Address Change/Moving
If you experience a change of residence, you may want to change your Exelixis benefits coverage. Note: If you submit an address change through Workday that changes your address to a different state, an address-change life event will be triggered automatically. You’ll see the benefit event in your Workday inbox and have 30 days to take action.
Note: Coverage or cancellation is effective as of the first of the month following your election-change request.
Medical, Dental, and Vision
You may enroll in a medical plan option available in your new location if your current plan is not available in that location, or you may enroll in a newly available plan option (e.g., Kaiser HMO). If you’re enrolled in the Kaiser HMO and your covered dependent moves, you may enroll yourself and your eligible dependents in another option that provides coverage for you and your eligible dependents.
You may not make changes to your dental or vision coverage.
Flexible Spending Accounts
You may not make changes to your Health Care Flexible Spending Account (FSA).
If your move affects your Dependent Care Flexible Spending Account (FSA) expenses, you may enroll, increase or decrease your contributions for the remainder of the plan year, or stop participating.

Managing an Illness or Injury
Sometimes a physical injury or medical condition can keep you from participating in your regular day-to-day activities. Here’s how being on a disability leave may affect your benefits.
Note: Coverage or cancellation is generally effective as of the first of the month following your election-change request.
Medical, Dental, and Vision
Your benefits will continue for six months following the exhaustion of your protected medical leave (i.e., FMLA). Your benefits will terminate on the last day of that sixth month.
During a leave, your benefit premiums will be deducted from your Exelixis pay. This will include sick time, vacation time, and short-term disability benefits. If you go on unpaid leave, you’ll need to complete a GHI and Pay Continuation Form with Larkin indicating whether you’d like the owed benefit premiums deducted from your paychecks after your return to work.
Flexible Spending Accounts
Your FSA deductions will continue to be deducted from your pay as long as you’re receiving pay from Exelixis.
Life and Accident Insurance
If you’re disabled for more than six months, you may be able to continue your optional life insurance coverage at no cost to you under a premium waiver, subject to the insurance carrier’s ongoing approval.
Voluntary Accident and Critical Illness Insurance
You can enroll or make changes to your voluntary accident and/or critical illness insurance.

We wish you well in your future endeavors. Here’s what you need to know to transition and/or terminate your Exelixis benefits.
Note: Cancellation of medical, dental, and vision coverage is effective as of the last day of the month in which your last day of work occurs. For all other benefits, cancellation is generally effective as of your last day of work.
Medical, Dental, and Vision
You have 60 days from when your coverage with Exelixis ends to enroll in COBRA. A COBRA packet will be mailed to your home address as it’s listed in Workday. The enrollment choices that you made for the current fiscal year will determine the type of COBRA coverage for which you (and your dependents) will be eligible. You pay for COBRA coverage on a monthly basis.
You may want to consider buying coverage through the health insurance marketplace. You could qualify for lower costs on your monthly premiums and lower out-of-pocket costs.
Flexible Spending Accounts
You may be eligible to continue your Health Care Flexible Spending Account (FSA) benefits under COBRA through the end of the fiscal year. You can’t incur Health Care FSA costs after your last day of employment unless you enroll in COBRA. A COBRA packet will be mailed to your home address as it’s listed in Workday.
Your Dependent Care FSA will stop accruing money on your last day of employment. You can continue to submit claims for expenses that occurred prior to your last day of work through the end of the FSA grace period (March 31 of the following calendar year). After that date, you’ll lose whatever remains in the account.
Life and Accident Insurance
Your basic and voluntary life insurance and accidental death and dismemberment (AD&D) insurance ends on your last day of work. You’ll have the option of converting all of these plans, except the voluntary AD&D plan, to an individual plan, subject to certain conditions. You’ll receive conversion information directly from Prudential.
Voluntary Accident and Critical Illness Insurance
If you’d like to continue your voluntary accident and/or critical illness insurance coverage beyond your employment at Exelixis, fill out and return to Prudential the continuation forms (included in the Additional Resources below) within 30 days of your last day of work.
